Coldstream Concrete has been fined $75,000 following an investigation into a workplace accident that cost a man his arm.

39 year old Geoffrey Morphew lost his arm August 2nd, 2017 when he was one of two workers helping to move the concrete culverts from a bed of the trailer onto the ground.

Morphew was rigging the concrete culverts to the crane from the ground, when it broke free from the crane and crushed his arm.

An investigation by the Ministry of Labour determined that a pin in the roller chain link connecting the crane’s hoist transmission drive socket to the hoist drum socket assembly failed. The ministry found that Coldstream Concrete failed to comply with the Occupational Health and Safety Act by not ensuring material would be prevented from falling. A 25 percent victim surcharge was also imposed.
